  1.  Does my plan include “out-of-network” coverage for mental health?

  2.  What is my annual deductible for mental health benefits?

  3.  What is my co-insurance? (What percentage does the plan cover?)

  4.  Is there a limit on the number of sessions my plan will cover per year?

Learn how to submit super bills

A super bill is a specific form that can be used to request reimbursement from your insurance. This can be auto-generated by my records system. I recommend contacting your insurance about the proper way to submit these bills for approval.
